Freedom of Information Request

As part of a legal case against me (Dr Tara Dorosti) the council officer Megan Thomas of the planning department submitted a document to the Swansea Magistrates Court, her reference MT6, an image of the document is attached. If you refer to the image it states "Document Generated 2017-12-08. Please supply the complete document from which this partial document MT6 submitted to the court is taken. A pdf via email will be fine.
